Program Details
Program includes complete health history, review of medications, previous weight loss programs attempted, current medications, review of sleep pattern, review of mood and mental health. Labs will be ordered on a case by case basis (insurance can be used for covering this cost).

​Initial Consultation -- review medical history, review or order labs (use insurance to cover cost), Biometric screening (to measure muscle mass, body fat, visceral fat, etc), waist measurements, Vitals.

Initial Medical Exam fee of $275 due at program start date.

Program Fee's: Larger Vials are more economical on monthly basis!

$400 per month ($400/vial) - Small Vial --> lasts new user about 4 - 6 weeks (approx 1 month)

$350 per month ($700/Vial) - Medium Vial --> lasts new user about 8-10 weeks (approx 2 months)

$300 per month ($1000/Vial) - Large Vial --> lasts new user about 12-16 weeks (approx 3-4 months).

Payment and Insurance

We will provide a “superbill” for insurance reimbursement.

Payment is due day of service with cash, check, credit card.

Glow Health Medical Clinic is an “out-of-network” provider for all private health insurance companies. Our office will gladly provide you with a billing sheet that you can submit to your insurance provider to possibly obtain out-of-network reimbursement.
At this time, we are not a participating provider with Medicare, but please subscribe to our mailing list and we will update you when this becomes available.
